Online Proof of Claim Filing Terms and Conditions: 


This online form is used to make a claim for payment in a bankruptcy case. Do not use this form to make a request for payment of an administrative expense. Make such a request according to 11 U.S.C. § 503.

Filers must omit (or redact) information on this form, or on any attached documents, that are entitled to privacy. Your Proof of Claim must not contain any type of Personally Identifiable Information (PII) or Protected Health Information (PHI). Examples of these types of information involve 1) Full social security numbers or tax identification numbers (only last four digits), 2) a complete birth date (only year), 3) a full financial account number (only last four digits), 4) confidential medical information/medical records 4) a minor’s full name (initials only). Attach redacted copies of any documents that support the claim, such as promissory notes, purchase orders, invoices, itemized statements of running accounts, contracts, judgments, mortgages, and security agreements.  

Completed Proof of Claim Forms and Supporting Documentation are public documents and will be publicly available and/or accessible on the Debtors case website that is being hosted by Reliable pursuant to Court Order. Additionally, such information may be shared with certain third parties affiliated with this matter in furtherance of the bankruptcy case and process. 

Neither Reliable nor the Clerk of the Court are responsible for reviewing any document for Personal Identifiable Information (PII), Protected Health Information (PHI) or Confidential Information and you hereby release and agree to hold harmless Reliable and the Clerk of the Court from the disclosure of any Personal Identifiable Information (PII), Confidential Information or Protected Health Information (PHI) included in your submission.



Use of the Website and Submission of Claim Data

You may use this website only for lawful purposes and in accordance with the Reliable Terms and Conditions (click here) and the Online Proof of Claim Terms included herein. A person who files a fraudulent claim could be fined up to $500,000, imprisoned for up to 5 years, or both. 18 U.S.C. §§ 152, 157, and 3571.
You agree not to use this website:

In any way that violates any applicable federal, state, local, or international law or regulation;
To impersonate or attempt to impersonate Reliable, a Reliable employee, another user, or any other person or entity;
To engage in any other conduct that restricts or inhibits anyone’s use or enjoyment of this website; 
To attempt to gain unauthorized access to, interfere with, or damage any part of the website, the server for the website, or any server, computer, or database associated with this website including uploading any malicious files or virus; or
To otherwise attempt to interfere with the proper functioning of this website.
To not submit a Proof of Claim using a script, program or any other method that does not involve a human being.


You agree that you are the Creditor, or authorized to file a Proof of Claim on behalf of the Creditor, and confirm that you have a reasonable belief that all Claim and Creditor Information is true and correct. The person completing this proof of claim must sign and date it. FRBP 9011(b). If you file this claim electronically, FRBP 5005(a)(2) authorizes courts to establish local rules specifying what a signature is.

All information, including information relating to your Claim Data, that is filed electronically, including through this Site, is subject to the same conditions and restrictions as a paper-based proof of claim filed with a Bankruptcy Court and is subject to court orders, rules, procedures and applicable law relating to the bankruptcy case in which Claims Data is filed.
